PRACTICE HOURS
The office is open Monday to Friday beginning at 9 am and closing at 5 pm on Tuesday, Wednesday and Friday. New patients are seen by appointment on Mondays between 1-6 pm and on Thursdays between 2-7 pm; appointments may be scheduled by telephoning the office during business hours and speaking with the receptionist.

TELEPHONE CALLS-QUESTIONS:
Patients are encouraged to call with questions prior to scheduling a surgical consultation, once a procedure has been planned and during the post-operative period. Our staff will answer your questions or, for more technical inquiries, will relay information to Dr. Kraft and respond to your concerns as professionally as possible and in the most timely manner available.

QUESTIONS REGARDING FEES, PAYMENTS & INSURANCE:
Dr. Kraft participates in most major insurance plans, meaning that the practice accepts insurance proceeds for covered services, subject to copayments and deductibles which are the patient's responsibility. If you have questions regarding your insurance coverage for plastic and reconstructive surgery, you may email the administrator with the appropriate insurance and contact information. We accept Visa, Mastercard, Discover and American Express for non-covered services. Financing applications are also available. A deposit is required in order to reserve a date for a cosmetic surgery procedure. Surgical fees are payable in full prior to the date of operation. Cosmetic surgery fees are individualized and are based upon the nature and extent of the procedure to be performed. Many cosmetic surgery procedures require the services of an anesthesiologist. Anesthesia and facility fees vary according to the length and type of procedure.

In accordance with Dr. Kraft's belief that there is no acceptable substitute for doctor-patient consultation, all cosmetic surgery consultations are complimentary. Patients scheduling reconstructive surgery consultation are required to obtain the appropriate insurance referral authorization from their primary care physicians, where applicable.

FORMS FOR YOUR CONVENIENCE:
When you arrive at the office, you will be given a lengthy patient information questionnaire to complete. A health insurance authorization form is also required for patients whose services are covered under the terms of their insurance contracts. Should a patient wish to apply for financing of cosmetic surgery procedures, an application form must also be completed. Under federal law, all patients, whether cosmetic or reconstructive, must read and sign a patient privacy act declaration. To reduce delays in seeing Dr. Kraft, all of these forms are available on line. Please feel free to print and complete these forms prior to your consultation.
